How to apply for the onsite CAVS exam

Application for the on-site administration of the Certified Administrator of Volunteer Services (CAVS) Examination at the Anaheim Marriott Hotel is being handled by Applied Measurement Professionals (AMP), the Examination administrator.

Criteria specified in the revised 2008 CAVS Candidate Handbook and Application apply to this offering. Note: The revised Handbook will be available after June 15. Visit www.aha.org/certification and click on the CAVS logo to download copy of the Handbook or call AMP at (913)895-4600 for hard copy. Mail your completed application with the required Examination fee to:

Applications must be received by AMP no later thanAugust 7, 2008. AMP will forward a confirmation letter by snail mail and email no later than two weeks before the Examination. If you do not receive a confirmation letter within two weeks of the Examination, call AMP at (913) 895-4600. Remember to bring any confirmation notice provided by AMP with you to the test site. The confirmation contains a unique identification number required to test. Late applications will be returned.

Examination fees are $275 for AHVRP members and $425 for non-members. If paying by credit card, fax your completed application to (913) 895-4651. Be sure to sign the application and check the box that describes the correct examination type: “I am applying for a special domestic administration” and fill in the date of the exam: “September 4, 2008.” Online application for special administrations is not available. Fees are non-refundable. There will be no on-site application.

Candidates confirmed for the Examination should arrive on time and must present two forms of ID. One form must have a current photograph and the second form must have a signature. Candidates who arrive late or without proper ID may not be seated. These Candidates may re-schedule the Examination to computer-based testing for a $100 fee by calling AMP.

Scores for the paper-and-pencil administration will be mailed to each candidate within 6-8 weeks of taking the Examination.

Any candidate with an ADA-recognized disability may request special accommodations by printing out the appropriate two pages of the revised Candidate Handbook and mailing or faxing them to AMP with the application and fee.
